Help me with costumer's report template

Hi everyone

I attached an image of one costumer's report template that wants to use Wialon services. I try to explain each Part, Column and Row of report template. Please check the possibility of provide the costumer such a report.
Regarding the attached image:

1. The column of Days of month
2. A column to add Text to it manually by operator
3. Begin Time of work for each day, Considering that Begin Time should insert for each day manually by operator
4. End Time of work for each day, Considering that End Time should insert for each day manually by operator
5. Moving Duration (Ride Duration) between Begin Time and End Time
6. Stop/Parking Duration between Begin Time and End Time
Note: End Time - Start Time = Moving Duration + Stop Duration
7. Mileage between Begin Time and End Time
8. A column to add Text to it manually by operator
9. Driver's Name, it can be in other table
10. Date of executing of report, it can be in other table
11. Month and Year (Date) of report, for example the report is related to 2015 April and each row is one of days, So we have i description for the Month, it can be in other table
12. Unit Name, it can be in other table
